LIABILITY WAIVER & RELEASE FORM

Agreement Regarding Participation, Assumption of Risk, Waiver and Release of Liability, and Indemnification

By signing this waiver, I acknowledge that I am voluntarily allowing my child to participate in physical exercise, training activities, and competitions associated with the 2025 WE RISE CLUB SEASON/TRAININGS at my own risk. This agreement serves as:

1. An acknowledgment of the inherent risks associated with participation.

2. A waiver and release of liability for all claims related to injuries or damages.

3. A consent for media use, allowing WE RISE SPORTS LLC & WE RISE 808 to use images/videos for promotional purposes.

ASSUMPTION OF RISK

I understand that participation in the 2025 WE RISE CLUB SEASON/TRAININGS involves inherent risks, both known and unforeseen, which may result in serious injury, disability, paralysis, or even death. These risks include, but are not limited to:

• Court/field conditions

• Contact with other players, referees, or spectators

• Weather conditions

• Improper techniques or training

• Malfunctioning or inadequate equipment

• The actions or negligence of coaches, referees, volunteers, teammates, or opponents

I, on behalf of my child, voluntarily assume all risks associated with participation and fully accept responsibility for any injury or damages that may occur.

I, the undersigned parent/legal guardian, along with my heirs, executors, and successors, waive and release all claims against WE RISE SPORTS LLC, WE RISE 808, NANAKULI HIGH SCHOOL, its directors, employees, volunteers, and affiliates arising from any injury, loss, or damage sustained during participation.

By registering my child with WE RISE SPORTS, I acknowledge and agree to the following terms:

1. Uniform Fees:

   I understand that the $110 uniform fee (jersey and shorts) is due upfront at the time of registration and is non-refundable.

2. Monthly Payments, Leagues, and Tournaments:

   I understand that monthly payments, league fees, and tournament costs are non-refundable, regardless of attendance or early withdrawal. When a player commits to participating in a league or tournament, costs are divided among all committed families. If a player withdraws after committing, it can affect game eligibility, team numbers, and financial obligations for the remaining families.

   • 💻 $65/month if paid online (includes Hawai‘i GET and processing fees)

   • 💵 $60/month if paid in cash

   • Leagues & Tournaments Prices will be given at time of commitment and will vary on amount of players participating.

3. Team Impact of Withdrawal:

   I acknowledge that withdrawing from the team or specific events after committing can negatively impact the team’s ability to participate in games, meet roster requirements, and maintain scheduling. I understand that my commitment supports not only my child’s development but the experience and fairness for the entire team.

By completing registration, I confirm that I have read and agree to the terms above, and I am committed to supporting my child’s participation and the success of the WE RISE SPORTS program.
